Researchers analyzing data from 396 GLP-1 users and 1,635 matched controls found that, on average, patients experienced a 9.8% reduction in total body weight over one year. While fat accounted for the majority of this loss, muscle tissue comprised approximately 21.6% of the total, a figure comparable to standard calorie-restricted dieting. However, the study emphasizes that these averages mask significant individual variability.
Muscle-to-Fat Ratio Predicts GLP-1 Weight Loss Outcomes
A study published in Mayo Clinic Proceedings: Digital Health reveals that a patient's baseline muscle-to-fat ratio is the strongest predictor of how their body composition changes during GLP-1 therapy. The findings offer a clinical tool to identify individuals at risk of losing muscle mass while using weight-loss medications.

Agathe Chabassier, an RWE Data Scientist at Withings who led the research, noted that patients starting with a higher fat-to-muscle ratio typically retain more lean mass during treatment. Conversely, those who are leaner at the outset are more prone to losing muscle. By tracking body composition through connected devices rather than relying solely on total weight, clinicians can detect unhealthy trends early and intervene through adjustments in resistance training or protein intake. These insights provide a critical framework for monitoring patients both during active therapy and in the period following cessation, when weight regain often occurs.
