Raintree Acquires Spike Technologies to Automate Physical Therapy Billing

Physical therapy practices face a 13% claim denial rate and mounting administrative strain, prompting Raintree to acquire Spike Technologies. By integrating agentic AI voice tools directly into its EHR platform, the company aims to replace manual, repetitive revenue cycle tasks with autonomous digital agents capable of complex decision-making.

The acquisition targets the most labor-intensive aspects of rehabilitation healthcare, specifically prior authorizations, payer communications, and claim follow-ups. Unlike traditional chatbots or rigid rule-based systems, Spike’s technology utilizes voice agents designed to understand clinical and financial context. This allows the AI to execute multi-step workflows rather than simply navigating fixed menu options. Raintree CEO Nick Hedges expects this shift to reduce revenue cycle management expenses to approximately 1%.

Because these tools are embedded within the existing Raintree EMR, providers avoid the friction of third-party integrations or system migrations. Povilas Gudzius, CEO of Spike, noted that the platform’s strength lies in its ability to reason through complex tasks at scale. The companies plan to detail their roadmap for autonomous revenue cycle management at the TherapyCon event in Austin this August, where they will present the potential for AI-driven operational efficiency.
